This form is to be completed by current and former staff who believe they are entitled to the transport allowance (known by its payroll code GTRP) for worked shifts, but who have not claimed them on their timesheets.
The transport allowance is provided for in clause 33.5 of the KiwiRail and RMTU collective agreement.
Please note: Former employees who left KiwiRail before 2 August 2015 are not eligible for remediation.
